Eligibility Information for Prospective StudentsPre-Admissions ServicesAll prospective students are encouraged to contact ODS in the early stages of their college planning. Pre-admission services include meeting with disability counselors to provide information about eligibility for academic support services and accommodations, appropriate documentation of disability, housing considerations, and transition issues. Verification forms for various disabilities are accessible on this website. If you prefer to type information, please click on the editable version of the appropriate verification form. Then, print it and make sure that the qualified diagnosing professional signs it before submission to ODS. The Admissions Application ProcessTo attend the Ohio State University and use ODS services, first complete an application to the University. The admissions application is also available on-line at www.osu.edu/futurestudents The admissions application process for students with disabilities is the same as that of other students. Disclosure of your disability is optional and voluntary. You may disclose your disability if you feel that your academic performance was adversely affected due to circumstances pertaining to your disability. If you choose to disclose, provide that information on a separate sheet of paper and attach it to the admissions application. It is advisable to send documentation directly to ODS. Your eligibility for services/accommodations from ODS is a separate process and is done independently of the Office of Admissions. After documentation is reviewed, ODS will send an email notification to your OSU email account, (e.g. miller.6789@osu.edu), acknowledging receipt of your documentation and your eligibility status. Prospective students that do not yet have an OSU email account will be notified via paper letter sent to their home address. Contact ODS early in the admissions process, particularly if you have questions about what documentation is required for eligibility purposes for certain disabilities. |
